Endoscopic Transsphenoidal Surgery: Factors Associated with Tumor Progression in Pituitary Adenomas

This study aimed to measure the incidence of tumor progression among patients that underwent endoscopic transsphenoidal surgery for pituitary adenomas, as well performance molecular and radiological factors are commonly associated with progression. Patients both adult pediatric populations adenomas who were treated first time in a single tertiary care center, between June 2006 December 2019, included. Clinical features, laboratory results, imaging findings, test results collected. Progression or recurrence was measured follow-up MRI. Eighty-eight 19.5% presented gonadotroph null-cell followed by corticotrophs (17.2%) somatotrophs (13.8%). 20.7% had cellular atypia, 26.2% p53 expression, up 79.5% Ki-67 under 3%. On postoperative MRI (available 90.9% patients), 43.8% residue. Tumor occurred 32 (36.4%). The median progression-free survival 5.37 years (95% CI = 3.29–N/A). Cellular elevation, cavernous sinus invasion, residue suggested significant prognostic factors. Nonetheless, multivariate time-to-event analysis identified only factor significantly progression: HR 4.0, 95% 1.56–10.31.
